Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
- Maintaining a clean, safe, and healthy environment within the client's residence. Duties may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing client's dishes or laundry, and other tasks to maintain cleanliness and safety for the client.
- The performance of all personal care activities contained in a written Plan of Care
- Monitor patient health by observing and recognizing changes in condition, and reporting findings accurately using Assisting Hands system.
- Support clients that could include specialized needs including Alzheimer’s care, dementia care, hospice care, and working with individuals with developmental disabilities or disabilities.
- Engage in home & community care activities that support clients’ independence while respecting their privacy and preferences.
- Abide by policies and procedures of Assisting Hands Santa Clara and CDSS guidelines
Hours: Dependent on assignment; Shifts available 24 hours a day, 7 days per week and based on availability. Assisting Hands does not guarantee hours.
Desired Experience
- Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments is highly preferred.
- Experience working with patients requiring hospice or dementia or memory care services is advantageous.
- Strong understanding of medical terminology relevant to patient care scenarios.
- Proven ability to handle sensitive situations with compassion while maintaining professionalism under HIPAA guidelines
