Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Provide education and support to patients and their families regarding health management.
  • Coordinate with interdisciplinary teams to ensure optimal patient care.
  • Document patient progress and maintain accurate medical records.
  • Assist with daily living activities and promote patient independence.
  • Provide training to  caregivers and corrdinate educational sessions for families. 
  • Ensure compliance with healthcare regulations and safety standards.
Requirements:
  • Current RN license in the state of North Carolina.
  • Minimum of 2 years of nursing experience, preferably in home Care or community care.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate demeanor with a patient-centered approach to care.
  • Basic Life Support (BLS) certification required.
  • Reliable transportation and a valid driver's license.

Compensation: $35.00 - $45.00 per hour
