🧡 Who We’re Looking For:

Required:

  •  Willingness to provide approximately 12 hours per week of direct client care 
  •  Strong organizational and problem-solving skills 
  •  Ability to manage multiple priorities in a fast-paced environment 
  •  Excellent communication and relationship-building abilities 
  •  Active STNA certification, current CPR, and TB test 
  •  Valid driver’s license with reliable, insured transportation 
Preferred:

  •  Background in Social Work, Nursing, or a related field 
  •  Experience in home care scheduling or caregiver staffing 
  •  Familiarity with AxisCare or similar home care software 
  •  Experience in home care, hospice, assisted living, or healthcare operations 
  •  Previous roles such as Care Coordinator, Case Manager, Discharge Planner, or Patient Advocate

🏡 Key Responsibilities:

đź“… Scheduling & Staffing Coordination

  •  Manage caregiver schedules using AxisCare 
  •  Ensure full client coverage and prevent missed shifts 
  •  Respond to call-offs and urgent staffing needs 
  •  Maintain a reliable backup caregiver pool 
  •  Monitor overtime and scheduling efficiency 
🤝 Client Care Coordination

  •  Match caregivers with clients based on skills and needs 
  •  Understand care plans and home environments 
  •  Support client satisfaction and caregiver retention 
  •  Serve as liaison between clients, families, caregivers, and providers 
🧑‍⚕️ Caregiver Support & Field Operations

  •  Address caregiver and client concerns 
  •  Resolve scheduling and care-related issues 
  •  Provide field support and maintain strong client relationships 
đź“š Staffing & Development

  •  Identify training needs and coordinate with clinical partners 
  •  Support recruitment efforts and address staffing gaps 
🌿 Hospice Coordination

  •  Assign qualified caregivers to hospice clients 
  •  Coordinate with hospice partners as needed 
đźš— Field Engagement

  •  Provide ~12 hours/week of direct client care 
  •  Assist with coverage and support in the field 
đź“‹ Compliance & Documentation

  •  Ensure documentation meets required standards 
  •  Confirm records are uploaded in AxisCare 
  •  Maintain complete caregiver files and regulatory compliance

Full-time role combining:

  •  Operations coordination 
  •  Caregiver scheduling 
  •  ~12 hours/week of direct client care
