- Â Active Ohio LPN or RN licenseÂ
- Â Hospice nursing experience requiredÂ
- Â Minimum of 1 year of clinical experience (hospice, home health, or related setting)Â

- Â Reinforce hospice-directed plans of careÂ
- Â Observe and report changes in patient condition to hospice teamsÂ
- Â Serve as a liaison between patients, families, caregivers, and leadershipÂ
- Â Respond to questions with empathy, clarity, and professionalismÂ
- Â Maintain accurate documentation and support compliance standardsÂ
