🩺 What We’re Looking For:
✔️ Active Ohio LPN or RN license
✔️ Hospice nursing experience required
✔️ Minimum 1 year of clinical experience (hospice, home health, or related setting)
✔️ Valid driver’s license with reliable, insured transportation
✔️ Strong communication skills and compassionate leadership mindset

📋 What You’ll Do:
🔹 Educate and guide caregivers on policies, documentation, and hospice best practices
🔹 Support and reinforce hospice-directed plans of care
🔹 Observe and report changes in patient condition to hospice teams
🔹 Serve as a clinical liaison between patients, families, caregivers, and leadership
🔹 Respond to questions with empathy and professionalism
🔹 Maintain accurate documentation and regulatory compliance
