The RN will be compensated per visit in the following increments:
- $125 for initial assessment/POC
- $15 per 30 day follow up call (these take approximately 5-7 minutes)
- $75 for 60 day visit/Caregiver Supervisory Visit
Performing client assessments
- Establish client care plans
- Maintain appropriate client and staff documentation/records (best practices) as per agency policy
- Make home supervisory visits to assess client status and caregiver performance
- Prepare performance evaluations for caregiver staff as required
- Strong clinical and assessment skills
- Strong interpersonal and customer service skills
- Excellent communication, organizational and mentoring skills
- Must be organized, detail oriented, self-motivated, and able to work independently
- Ability to handle multiple priorities simultaneously
- Prior community health experience, long-term care, or case management experience required.
