About the Role
We’re looking for a compassionate and self-motivated Registered Nurse (RN) Case Manager to provide in-home visits and case management for clients throughout Cuyahoga County. What You’ll Do
- Visit clients in their homes to provide assessments and wellness checks
- Create and follow individualized care plans that meet each clients unique needs
- Partner with families, physicians, and other care providers to ensure seamless coordination
- Offer education and encouragement to clients and caregivers
- Keep clear, accurate records that reflect the quality of your care
- Bring warmth, empathy, and professionalism to every visit

What We’re Looking For
- Active RN license in Ohio
- At least 1 year of clinical experience (home health or case management preferred)

Compensation: $40.00 per hour
