This full-time position is ideal for an experienced clinician (such as an LPN or Social Worker) who is passionate about both care coordination and business growth. In this role, you’ll be a part of our care management team to oversee and enhance our non-skilled home care services, while also playing a key part in expanding our agency’s reach within the community.
