- Build and maintain strong relationships with hospitals, rehab centers, clinics, discharge planners, senior centers, and other referral sources
- Conduct educational presentations for professionals and families on topics like dementia care, Parkinson’s care, fall prevention, and aging in place
- Represent Assisting Hands at health fairs, networking events, and conferences
- Track outreach activities and leads using our CRM software
- Schedule and complete weekly visits with referral sources
- Coordinate with internal staff to ensure quality of service and client satisfaction
- Participate in strategic growth meetings and occasional on-call rotations
- Share community feedback and referral trends with leadership team

Required: Passion for senior care and community engagement

Preferred: LMSW, LBSW, or background as RN, LPN, PT, OT, or in healthcare/social services
- Excellent verbal communication and presentation skills
- Organized, proactive, and relationship-driven
- Proficient with Microsoft Office (especially PowerPoint) or Canva
- Prior experience in healthcare marketing, home care, or community education a plus
- Familiarity with CRM systems (or willingness to learn)
- Reliable transportation and ability to travel locally
