At Assisting Hands Home Care – Polk County, we’re looking for a Care Manager who is service-oriented, empathetic, and detail-minded. This role is a great fit for someone who enjoys managing a variety of cases, building meaningful relationships, and being part of a team that genuinely cares about improving the lives of seniors.

The Care Manager is the primary relationship point for clients and their families. You will participate in developing the plan of care, recommend updates based on client needs, and communicate changes to the RN and Client Care Coordinator team. You’ll conduct care management visits, support caregivers, and help ensure clients receive consistent, high-quality service. While you’ll assist with some inquiries and intakes, the majority of this role centers on active care management and ongoing client support.

This position requires flexibility. Client needs don’t always fall within business hours, so the ideal person is comfortable taking calls when situations arise and responding with urgency and empathy.

What You’ll Do

  • Serve as the primary contact for clients and families

  • Conduct care management visits to assess needs and service quality

  • Participate in developing the plan of care and make recommendations to the RN

  • Communicate care updates to Client Care Coordinators

  • Provide coaching and support to caregivers

  • Assist with new client intakes when needed

  • Strengthen referral relationships tied to active clients

  • Maintain accurate documentation in our care management platform

What We’re Looking For

  • Active RN, LPN, or LCSW license in Florida

  • Experience in home care, care management, or case management preferred

  • Strong relationship-building and communication skills

  • Able to manage multiple needs with urgency and attention to detail

  • Comfortable with technology and care management software

  • Passionate about serving seniors and supporting families

Schedule & Compensation

  • Office hours: 8:30 AM – 5:30 PM

  • Evening/weekend availability when client needs arise
